Pre-Participation Examination Form

Hoopeston Area's annual sports physical clinic at CARMC on May 22nd from 12:30-3:30 pm.  If parents will not be there with the child, the child will need this form signed by their guardian in order to be seen.  It is recommended to call the clinic ahead of time and set up a time, but you will not be turned away if you arrive without one.  The number to the clinic is 217-283-5644. 

Just a reminder that incoming 6th and 9th-grade students do not need a sports physical.  They will need a full school physical that will count as their sports physical for the year.
